Hi,

Few queries, if you can help:

1. Does the birth certificate copy needs to be attested by a Gazetted officer?

2. How much did you have to pay for Annexure I and from where in Pune did you get it done?

Hello Praveen,

(1) No need to attest the birth certificate

(2) I payed around Rs 150/- for the Annexure I and got it from a Notary at Hadapsar, Pune.

Question 10. In case of a minor child passport, whose father is abroad, whether consent is to be obtained to apply for a passport? If so, what is the procedure?

Ans. Yes. Consent of the spouse, who is abroad, is required for submitting the application. It should be in the form of a Sworn Affidavit duly attested by the Indian Embassy/Consulate abroad.

Hi,

I have doubt regarding my daughter passport application.
She is just 20 days old. i want to apply for her passport. everything is ok but i do have cencern about the address.
i have recently moved in pune last oct. previous i was in Germany for around 2 yrs. currently staying in rented house since oct 2012.
will this affect for passport approval ?

Hi Shekhar,

Since it is a minor's passport who was born in India, I think you should present proof of your current residence in Pune (since October 2012) as well as a proof that you were outside India for last two years. This should suffice.

As you mention :
"if mother's name on the passport is the maiden name but the minor's birth certificate has her new name then in the passport application for the minor you will still need to give the same name as on her passport (dont use her new name anywhere in the application for minor's passport)."
question is Which mother name will apear on Minor's passport ( as on birht cert. or as on mother's passport ??

Is it mandatory to take your kid to the passport office? I read somewhere that it is not mandatory to take your kid there... but only his/her passport size picture.

Hi Sandeep, Annexure I is to be filled by the applicant, so it has to be in baby's name. However, it has to be notarized, so notary will take thumb impression of the child and the notary will ask to paste your photograph as well as your spouse's photograph on the application and ask you to sign across it and then notarize it. This is what happened in my case.
